Objective: The goal of this study was to examine if there is an increased incidence of depression between left sided cerebral hemorrhagic strokes and left sided cerebral ischemic strokes and determine if a depression screen should be implemented as a standard of care for the stroke population evaluated in this study as well as a depression severity comparison with respect to the type of cerebrovascular event.
Design: Prospective Study Setting : Inpatient Rehabilitation Unit Participants : 130 post-stroke patients
Interventions: The PHQ-9 will be administered to patients with left sided ischemic stroke and hemorrhagic stroke on an acute inpatient rehabilitation unit within 24 hours of admission and repeated within 24 hours of discharge.
Main Outcome Measures: PHQ-9 Score Treatment Summary as per the Center for Quality Assessment and Improvement in Mental Health (CQAIMH), Brief Interview for Mental Status (BIMS) on admission for post-stroke patients.
Results: In patients with ischemic strokes, there was a significant difference in PHQ-9 admission scores (M=3.57, SD=4.28) and PHQ-9 discharge scores (M=32.23, SD=2.85); t(112)=4.69, p = .001. There were no significant differences in PHQ-9 admission scores in patients with a left-side stroke and patients with a right-side stroke. Additionally, there were no significant differences in PHQ-9 discharge scores in patients with a left-side stroke and patients with a right-side stroke. Conclusions: The results demonstrate that there was no statistically significant difference in the PHQ-9 score on admission or on discharge with respect to laterality (left vs. right). However, there was a statistically significant improvement in the PHQ-9 scores within the ischemic strokes, independent of laterality. This data shows the importance of post-stroke depression screening within the inpatient rehabilitation setting, specifically using the PHQ-9 Score Treatment Summary.
