Objective: To evaluate the safety of ultrasound-guided carpal tunnel releases (USCTR) for the treatment of carpal tunnel syndrome (CTS)
Design: Retrospective chart review Setting : Single-site tertiary care academic medical center Participants : Thirteen subjects who underwent elective USCTR
Interventions: USCTR performed by a single investigator (JS) using a commercially available device (SX-One MicroKnife)
Main Outcome Measures: Complications associated with the procedure and medical comorbidities
Results: Seventeen CTR performed on 13 subjects were included in the study. No complications were reported. The average age and body mass index were 60 (37 to 87) and 30.96 kg/m2 (24 to 39.44 kg/m2), respectively. There were seven (54%) males and six (46%) females. Nine (53%) procedures were performed on the left and eight (47%) were performed on the right. All procedures were performed under sterile technique in the operating room. Comorbidities included hyperlipidemia (six [46%]), hypertension (six [46%]), diabetes mellitus type II (one [8%]), tobacco use (one [8%]), transient ischemic attack (one [8%]), chronic kidney disease(one [8%]), history of colon cancer (one [8%]), history of breast cancer (one [8%]), coronary artery disease (one [8%]), obstructive sleep apnea (one [8%]), hypothyroidism (one [8%]), gout (one [8%]), psoriasis (one [8%]), asthma (one [8%]), and atrial flutter (one [8%]). Previous steroid injection(s) to the area included none (eleven [65%]), one (three [18%]), greater than three (one [6%]), and unknown (two [12%]). Conclusions: This study suggests that USCTR with the SX-One MicroKnife is safe when performed by a physician with expertise in ultrasound-guided musculoskeletal procedures. As there were no complications and minimal comorbidities, we were unable to perform a sub-analysis to determine if there was an association between comorbid factors and increased complication rates. Additional research would expand the results of this initial experience.
