The Use of Alpha-2-Macroglobulin as a Regenerative Treatment for Patients with Chronic Pelvic Pain Syndrome

Objective: The purpose of this study was to determine the effectiveness of a local alpha-2-macroglobulin (A2M) injection in alleviating pain and improving function in patients suffering from chronic pelvic pain syndrome (CPPS).

Design: Retrospective chart review Setting : Secondary care, outpatient private practice Participants : 19 participants, 8 male and 11 female, with CPPS who had previously demonstrated a 30% improvement on the Visual Analogue Scale (VAS) from prior treatment, according to the Pelvic Rehabilitation Medicine (PRM) protocol, and did not have an associated malignancy or pregnancy.

Interventions: Ultrasound guidance was used to inject autologous A2M obtained at the same visit to Alcock’s canal bilaterally.

Main Outcome Measures: Pain and function were measured in each patient using the VAS and Functional Pelvic Pain Scale (FPPS), respectively. This was done before treatment, as well as at a follow up visit 5 – 19 weeks after the A2M injection.

Results: Improvement was appreciated for all individual categories of pain and function, except for running. Statistically significant (α = 0.05) differences were seen for walking, which improved, and for total function, which did not. Conclusions: This study has promising results regarding the use of localized alpha-2-macroglobulin as a novel, regenerative, non-opioid treatment option for patients with CPPS.
