The Effect of High Intensity Interval Training on Platelet-rich Plasma Composition

Objective: To determine the effect of high intensity interval training on platelet-rich plasma composition.

Design: Controlled Laboratory Study

Setting: Academic Sports Medicine Clinic / Comparative Orthopedics Laboratory

Participants: 5 men, 5 women, healthy with no major medical problems including inflammatory disease, cardiopulmonary problems or orthopedic conditions that would prevent participation in exercise.

Interventions: All subjects had a 15-mL blood draw to create PRP using the Arthrex ACP system. Immediately after the first PRP preparation, all subjects underwent a monitored exercise protocol that consisted of a 5-minute warm-up, followed by the original protocol described by Dr. Izumi Tabata (20-second sprint, 10-second rest for 4 minutes on a Schwinn AirDyne). Subjects were allowed to recover for up to 5 minutes. Then a second blood draw was performed to create post-exercise PRP. All PRP created was sent to the laboratory for analysis.

Main Outcome Measures: Pre-exercise and post-exercise PRP was examined for differences in platelet count, leukocyte count, differential, red cell count and mean platelet volume using an automated hemo-analyzer.

Results: Average age of participants was 39.6 ± 9.8 years with a range of 28 to 60. Average time to completion of the blood draw was 4 minutes and 36 seconds with a standard deviation of 21.5 seconds. Platelet count increased from 367.4 ± 57.5 to 497.7 ± 93.3 (P value .000). Mean platelet volume (fL) increased from 7.9 ± 0.6 to 8.3 ± 1.0 (P value .013). Red blood cells increased from 0.21 ± 0.16 to 0.30 ± 0.16 (P value .019). White blood cells increased from 2.4 ± 1.2 to 6.1 ± 4.5 (P value .010) All cell counts are presented in values of 1000/uL.

Conclusions: A brief 4-minute protocol of high intensity interval exercise is able to increase platelet yield in PRP. Further research is needed to determine if this increased cell count improves clinical outcomes.
