Objective: To compare range of motion, pain, and function in patients with chronic refractory Adhesive Capsulitis (AC) after TENEX® ultrasound (US) guided Coracohumeral Ligament (CHL) release vs control
Design: Randomized Controlled Trial
Setting: Outpatient Pain Clinic
Participants: 48 subjects (39 female, 9 male) with chronic AC refractory to PT and local steroid injection
Interventions: Patients were randomly assigned to percutaneous CHL release and CHL LA injection control
Main Outcome Measures: Pre/post-procedure external rotation and abduction were the primary outcome measures and VAS (Visual analogue Pain Scale) was secondary
Results: At one month follow-up mean pre-procedure VAS was 8.1 and the mean post-procedure VAS was 2.7. In addition, the average pre-procedure external rotation was doubled at one month from 30.15 degrees to 59.4 degrees. Likewise, pre-procedure abduction was 58.2 and 74.4 degrees at one month follow-up. There were no adverse events.
Conclusions: Percutaneous US-guided CHL release with a TENEX® tissue remodeling tool improved pain, function, and ROM at one-month FUP. However, longer-term durability needs to be assessed to understand the true clinical importance of the procedure
