Outcomes of a Comprehensive Pain Rehabilitation Program on Fibromyalgia Patients

Objective: Fibromyalgia Syndrome (FMS) is a complex pain disorder that can be challenging to diagnose which is associated with many medication prescriptions. Our aim is to compare outcomes of fibromyalgia patients before and after Pain Rehabilitation Center.

Design: A retrospective study of Mayo Clinic Comprehensive Pain Rehabilitation Program (MCPRP) outcomes was conducted at admission and dismissals using self-reported questionnaires and medications.Setting : Mayo Clinic offers a 3-week outpatient Pain Rehabilitation Program which provides cognitive-behavioral interdisciplinary intervention to patients with chronic pain conditions including fibromyalgia.Participants : The retrospective study was conducted in FMS subjects who completed the 3-week MCPRP from 05/01/14 to 05/01/15 in Florida. Patients were divided in 2 groups according to their opioid consumption at admission: Non Opioids and Opioids.

Interventions: The MCPRP has an emphasis on physical reconditioning, cognitive-behavioral interventions and medication weaning that consists of physical therapy, occupational therapy, cognitive behavioral therapy and medication management.

Main Outcome Measures: Upon enrollment into the MCPRP, patients provide a detailed list of their current medications. Opioid doses were recalculated into oral morphine equivalents (OME). Surveys used in this study include: Pain scores, Pain Self-Efficacy, Center for Epidemiologic Studies Depression Scale, Multidimensional Pain Inventory, SF-36 General Health Perceptions, Pain Catastrophizing Scale.

Results: After completion of the program, all questionnaires outcomes significantly improved in both groups, including pain score, SF-36, PSEQ, PCS, and CES-D scales. The total number of medications was decreased by dismissal from the program in both groups, including antidepressants, benzodiazepines and supplements/vitamins. In the opioid group, anticonvulsants, NSAIDs, opioids, muscle relaxants were also significantly reduced at dismissal. In addition, LME was significantly reduced in the opioid group but not the non-opioid group.Conclusions: FMS patients experience marked clinical improvements in pain severity, physical and emotional health, and functional capacity when treated with a comprehensive pain rehabilitation with concomitant medication tapering including opioids.
