Efficacy and Sustainability of Ultrasound Guided Platelet-rich Plasma Injections in Reducing Sacroiliac Joint Disability and Pain

Objective: To determine the efficacy and sustainability of ultrasound guided platelet-rich plasma (PRP) injections in reducing sacroiliac joint (SIJ) disability and pain.

Design: Prospective cohort trial with follow-up at 2 weeks, 4 weeks, 3 months, and 6 months.

Setting: Private practice sports medicine clinic.

Participants: 50 subjects between 18 and 80 years old with chronic low back pain and sacroiliitis diagnosed via history and provocative physical exam tests.

Interventions: A single ultrasound guided PRP SIJ injection.

Main Outcome Measures: The primary outcome was disability reduction via Oswestry Disability Index (ODI). The secondary outcome was pain relief via 10-point Numeric Rating Scale (NRS).

Results: Significant reductions at 6 months post-injection were seen in ODI (M -9.70%; 95% CI [-6.06, -13.52]) and NRS (M -1.9%; 95% CI [-1.14, -2.78]) when compared to baseline. All times frames showed significant reduction in disability and pain compared to baseline. However, overall improvement in disability and pain diminishes after 4 weeks with no statistically significant reduction from 4 weeks to 3 months or 3 months to 6 months.

Conclusions: Ultrasound guided PRP injections in the SIJ are effective at reducing disability and pain with the majority of improvement seen within 4 weeks post-injection and with sustained reduction at 6 months post-injection.
