Effects of Trunk Stabilization Training Robot on Postural Control and Gait in Patients with Chronic Stroke: A Randomized Controlled Trial

Objective: The objective of our study was to prove the therapeutic effects of using a trunk stabilization training robot (3DBT-33) on patients with chronic stroke.

Design: Randomized Controlled Trial

Setting: From March 2017 to December 2017, patients with chronic stroke who were registered at two national university hospitals in Korea were included by convenience sampling. Sampling, as available, was done among all patients with stroke who were visited to the outpatient clinics.

Participants: The inclusion criteria were (1) patients who had experienced their first stroke at least 3 months prior; (2) the first episode of unilateral stroke (infarction, hemorrhage) included hemiplegia; and (3) patients with the ability to stand and walk with assistance (Berg Balance Scale: 21-40).

Interventions: The robot group (n=19) received 30 minutes of trunk stability robot training in addition to conventional physical therapy. The control group (n=19) received the same amount of conventional physical therapy as the robot group.

Main Outcome Measures: All participants were assessed by: the Functional Ambulation Categories (FAC), Timed Up and Go test (TUG), the Berg Balance Scale (BBS), the Korean Modified Barthel Index (K-MBI) and the Fugl-Meyer Assessment of lower extremity (FMA-LE).

Results: In both groups, there were statistically significant improvements in all parameters at follow-up assessment after 4 weeks of intervention (P<.05). There were statistically significant differences in FMA-LE, K-MBI and BBS between the robot and control groups (P<.05). There was no significant difference in FAC (P=.935) and TUG (P=.442). It showed more changes over the MDC in FMA-LE and BBS.

Conclusions: The findings in the present study showed that trunk stabilization rehabilitation training using a rehabilitation robot in patients with chronic stroke was effective to improve the balance and functions in lower extremities.
