Characterization of Physical Function and Cancer Related Physical Impairments in Hispanic Women with Breast Cancer

Objective: To report characteristics of Hispanic women with breast cancer with respect to physical function, body mass index (BMI), and depression.

Design: Retrospective, cohort study.

Setting: The patients has been receiving care in the cancer treatment center serving a large Hispanic population in the southeastern part of the United States. The patients were either actively undergoing treatment or had completed breast cancer treatment. Records of patients admitted between January 2019 and August 2020 were included for analysis. Data from patients in the study was extracted from their charts and entered into Excel spreadsheet for analysis.

Participants: This study included 322 Hispanic women with breast cancer.

Interventions: None

Main Outcome Measures: Physical function and fatigue were assessed using the Patient Reported Outcomes Measurement System (PROMIS) – Physical Function short form (PF) and PROMIS-Fatigue (F) short form. In addition, Timed Up and Go (TUG) test, Sit-to-Stand in 30 seconds (STS30) test, 4-stage balance test (4SB) and grip strength (GS) were measured. Depression was obtained from chart review.

Results: The mean PROMIS-PF and PROMIS-F of the sample was 39.8 and 54.7, respectively. The mean TUG was 10.4 seconds, STS30 was 11.2 times in 30 seconds, and GS was 19 kg, and 30.8% had impaired 4SB test. Nearly 40.8% were obese, 35.0% were overweight, and 20.8% had depression. Compared to normal BMI patients, the mean PROMIS-F score was significantly higher among overweight (P < 0.05) and obese (P < 0.05) patients. The mean STS30 score was significantly lower in obese patients, compared to normal BMI patients (P < 0.05). The PROMIS-PF, STS30 and GS were significantly higher while PROMIS-F and TUG significantly lower among depressed patients.

Conclusions: Hispanic women with breast cancer often have substantial loss of physical function, and this was exaggerated if they are obese, overweight, or depressed. Clinicians caring for this population should screen them for the presence of loss of physical function, BMI, and depression.
