Objective: Patients undergoing cancer rehabilitation have high rates of unplanned transfers. A majority of prior studies have been done in tertiary cancer center settings with the CMS diagnostic criteria waiver. The objective of our study was to determine risk factors that impact unplanned transfers (UPT) to acute care in patients with cancer undergoing inpatient rehabilitation.
Design: Retrospective Cohort Study
Setting: Freestanding Academic Acute Inpatient Rehabilitation Hospital
Participants: 330 admissions to oncology services in an acute inpatient rehabilitation (AIR) hospital.
Interventions: Data from initial evaluation upon inpatient rehabilitation admission, prior acute care admission, and ambulatory visits
Main Outcome Measures: Rate of and reasons for unplanned transfers to acute care.
Results: Overall rate of unplanned transfers was 36%. Factors associated with increased odds of UPT include: infection as reason for acute care admission (OR 2.2, P =.02), gastrointestinal malignancy as primary cancer (OR 3.5, P =.004), recurrent cancer (OR 3.7, P<.0001), development of cachexia in acute care (OR 2.1, P =.01), acute care length of stay >/= 10 days (OR 2.5, P =.0002), parenteral nutrition (OR 4.2, P =.01), graft vs host disease (OR 10.4, P =.04), pressure ulcers (OR 2.4, P =.001), admission creatinine.
Conclusions: We identified several novel factors associated with unplanned transfer from rehabilitation in cancer patients. These factors include cancer recurrence, cachexia, low creatinine, and poor 6 minute walk test. These findings will provide a framework for creating predictive tools for unplanned transfers in cancer rehabilitation patients.
