Assessment of Resident Understanding the Role of Skilled Rehabilitation Services and Patient Mobility in the Acute Care Setting

Background and/or Objectives: Promoting early patient mobility and optimal utilization of skilled rehabilitation services through resident physician education and implementation of the Activity Measure for Post-Acute Care (AM-PAC)

Design: Quantitative survey

Setting: Academic medical center

Participants: Internal medicine and general surgery resident physicians

Interventions: Quantitative survey given to resident physicians before and after an educational presentation is given by a physical medicine and rehabilitation resident physician, physical therapist and occupational therapist.

Main Outcome Measures: Comparison of answers given on a survey before and after an educational presentation about skilled rehabilitation services and AM-PAC

Results: Following a review of Physical Therapy (PT) and Occupational Therapy (OT) referrals over a 6 month period in 2018, 39% of PT referrals and 37% of OT referrals demonstrated suboptimal utilization of resources. This led to implementation of the AM-PAC form at our institution. Percentage of referrals with AM-PAC scores of greater than or equal to 23 decreased from 16.83% to 13.17 % from 2018 to 2020. This led to an educational intervention to elucidate a lack of understanding of the role of acute care skilled rehabilitation services. There was a 90.9% improvement in familiarity with AM-PAC after the presentation which was significant (p value < 0.0001). There was also a 12.5% improvement in resident physicians feeling comfortable promoting patient mobilization post-presentation. Unfortunately, no improvement was observed in understanding the specifics of skilled rehabilitation services such as the differences between acute versus subacute rehabilitation and best utilization of occupational therapy services. Interestingly, resident physicians subjectively felt more confident in understanding when an occupational therapy consult was ideal (2.49% improvement) post-presentation but when asked an objective question about occupational therapy, no improvement was observed.

Conclusions: The preliminary data is promising but the presentation will need modification to address its deficiencies. Further educational benefits may be elucidated as more data is collected with time.
