A Novel Mixed Reality System to Manage Phantom Pain In-Home: Results of a Pilot Clinical Trial

Objective: To investigate the pain and functional outcomes of using a Mixed Reality System for Managing Phantom Pain (Mr. MAPP), a newly developed system for in-home use, in a pilot sample of patients with unilateral lower limb amputation and phantom pain.

Design: Pilot clinical trial.

Setting: In-home intervention

Participants: 11 patients recruited from the VA North Texas Health Care System that fulfill study criteria

Interventions: Mr. MAPP, a system that mirrors the intact limb onto the amputated limb in a virtual environment, where patients can engage in interactive exercise games (exergames) concentrating on large limb movements, was the primary intervention. Study participants used Mr. MAPP to perform prescribed daily exercises in their home for 1 month.

Main Outcome Measures: Outcome measures collected at baseline and 1-month included: McGill Pain Questionnaire, Patient Specific Functional Scale, Brief Pain Inventory, Numerical Rating Scale, patient satisfaction survey, and exercise diary.

Results: In-home use of Mr. MAPP system was associated with a 20-35% decrease in pain intensity and a 45% increase in the ability to perform self-selected functional activity goals, over the 1-month period of in-home use. There was also a modest decrease in pain found immediately after using the system.

Conclusions: This pilot study suggests that in-home use of the Mr. MAPP system has potential to decrease pain and improve function in patients with lower limb amputation and phantom pain. Self-management and on-demand use are attractive features of this system. An adequately powered clinical trial is planned.
