Case Diagnosis: Application of mobile technology for GMFCS 5 cerebral palsy patient empowers caregivers and decreases pain and spasticity.
Case Description: 13 year old male with a history of of spastic quadriplegic CP (dorsal rhizotomy in 2013) who had years of rights hip dysplasia pain secondary to hip subluxation underwent bilateral adductor and iliopsoas tenotomies and bilateral proximal femoral osteotomies on 3/8/2021. Upon admission to Blythedale Children’s Hospital on 4/8/2021 for comprehensive rehabilitation, patient was continued on home baclofen but required increasing doses due to worsening pain and spasms in his hips, legs and arms. Standing motrin was added, but patient continued to require valium daily.
Setting: Inpatient rehabilitation unit at Blythedale Children’s HospitalAssessment/
Results: The patient had improvement in leg abduction from 30 degrees to 85 degrees on the right and 30 degrees to 60 degrees on the left, helping with groin hygiene. Elbow stretching exercises were incorporated, and after 5 days of compliance, the right elbow extension improved by 60 degrees and left by 80 degrees. Ever since the patient started the exercise regimen, pain and spasms improved allowing for discontinuation of standing motrin/tylenol and valium PRN.
Discussion: In efforts to decrease pain and spasticity medications, the authors utilized a novel technological application called Edenos that empowers the patient’s caretaker to partake with follow along stretching video exercises for CP patients. The app sends push notifications on a daily basis in order to remind the user to continue their exercises and track their improvement over time.
Conclusion: This case report demonstrates that there is improved pain control, minimizing the usage of pain/spasticity medications through the application of consistent standardized stretching exercises through Edenos. This decreases the usage of pain medications (reducing medical side effects) and prevents contractures, potentially staving off the necessity for further repeat surgeries and allowing for healthcare cost savings in the long-run.
