Case Diagnosis: A 57-year-old male with a 20-year history of left ring finger pain.
Case Description: This patient was referred to chronic pain management by orthopedics following multiple failed surgical attempts at symptom control of a traumatic neuroma. He suffered a traumatic event with subsequent amputation 20 years prior, along with several post-operative complications. As a result, he underwent five surgeries, none of which provided long-term resolution of pain. He failed numerous medications, such as gabapentin, amitriptyline, duloxetine, and pregabalin. He then underwent a diagnostic local nerve block with significant benefit but declined any chemodenervation treatment options. After undergoing several diagnostic local blocks with 100% temporary relief, the decision was made to progress to radiofrequency ablation (RFA).
Setting: Outpatient Pain Clinic at an Academic Institution.Assessment/
Results: Four weeks following RFA, the patient noted decreased hypersensitivity of the distal ring finger with approximately 50% relief of his overall pain. At nine months, he noted continued overall benefit with approximately 75% relief. He enjoyed significant functional improvement in the use of his left hand and was even able to return to recreational golfing.
Discussion: This case study is relevant to the literature as support for the use of RFA in treatment of pain caused by traumatic neuroma following unsuccessful surgical intervention.
Conclusion: RFA is a successful treatment option for pain due to neuroma following amputation as well as Morton’s neuroma. This case presents successful management of a traumatic neuroma in the hand.
