Objective: Diagnostic hip injections are used to guide management in adolescents and young adults with hip conditions, with the goal of confirming an intra-articular source of pain. This study describes the changes in provocative hip maneuvers, range of motion (ROM), and pain scores based on response to diagnostic hip injection.
Design: Prospective studySetting : Academic medical center, outpatient procedural clinicParticipants : Patients age 12-40 scheduled for diagnostic hip injection
Interventions: Fluoroscopically guided, intra-articular hip injection
Main Outcome Measures: Passive hip ROM (hip flexion (HF), HF internal rotation (HFIR), and HF external rotation (HFER)) and log roll, flexion adduction internal rotation (FADIR), and flexion abduction external rotation (FABER) were performed pre- and 10 minutes post-injection. Numeric pain scores (pre- and post-injection) and overall percent improvement were recorded 10 minutes post-injection. Based on reported overall percent improvement of pain, patients were categorized as nonresponders ( < 50% improvement), partial responders (50-75% improvement), and responders (>75% improvement).
Results: 50 patients (56 hips) were enrolled (mean age 18.5 years (SD 5.2), 88% (44/50) female). Average post-injection improvement in numeric pain score was 4.3 points in responders, 2.7 in partial responders, and 0.9 points in non-responders (SD 1.7, 1.1, 1.5, p0.05).Conclusions: In this cohort of adolescents and young adults with hip pain undergoing diagnostic hip injection, log roll and FADIR were the provocative hip maneuvers that showed a significant difference in pain improvement pre- vs post-injection between those considered responders, partial responders, and nonresponders. Change in range of motion pre- vs post-injection did not have statistical difference between the three groups, and is unlikely to be useful in interpreting response to diagnostic hip injection.
