Objective: Recognizing that disparities in health are largely influenced by social determinants of health—economic stability, education, social and community context, neighborhood and the built environment, and health care access and quality—we sought to: (1) examine the research on rehabilitation services provided in outpatient settings that identify and respond to patient’s unmet social needs, and (2) describe the research and practice-based models of integrating rehabilitation and community-based social services.
Design: A systematic review was conducted according to Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. Setting : Two investigators conducted a structured search of the databases PubMed, CINAHL, and Web of Science and the SIREN Evidence and Resource Library from inception through publication by September 30, 2019. Terms for each search were related to social determinants, risk, social needs and socioeconomic factors, setting of care, and screening, navigation, or referral. Articles were imported into Covidence for systematic screening and evaluation. Participants : N/A
Interventions: N/A
Main Outcome Measures: Characteristics of the care models integrating rehabilitation services and community-based organizations, social needs assessed and addressed, and subsequent patient outcomes.
Results: The search yielded 1229 unique studies of which 10 included rehabilitation services. No studies examined the implementation of community-based care models that integrated rehabilitation services and community social service organizations to address unmet social needs. Although many studies among patients with rehabilitation needs acknowledge the influence of social determinants on health and recovery, research evidence was not identified on interventions or service delivery models that directly sought to address social needs. Conclusions: Although social determinants of health have a role in rehabilitation outcomes, recovery from injury and illness, and prevention of functional decline, interventions or service delivery models in outpatient rehabilitation settings have not yet been studied and are greatly needed.
