Objective: The objective is to assess feasibility of an interdisciplinary intensive lifestyle medicine program for successful completion by patients with medical and behavioral health co-morbidities that enables them to meet their endpoint goals of having joint replacement surgery or improve function and health in managing low back pain (LBP).
Design: Patients enrolled in a 90-120 day intensive interprofessional lifestyle medicine program were followed to assess completion of out-come measures, program completion, and achievement of their specific end of program goals. Setting : Tertiary university Participants : Each patient had a primary goal to improve their overall health and function enabling them to undergo joint replacement surgery or to improve function and management of their LBP.
Interventions: Disciplines were assigned to each patient based on their screening information and the patient’s identified primary outcome goals as assessed by the physiatrist. Services provided included: behavioral health counseling, nutritional counseling, physical therapy, acupuncture, medical massage, aerobic exercise education, and smoking cessation.
Main Outcome Measures: Outcome measures included: completion of the program, goal achievement, and completion of outcome tools including: PROMIS anxiety, depression, pain interference and function domains, blood pressure, body mass index, C – reactive protein, hemoglobin A1C, and vitamin D level.
Results: Forty-one patients (20 females) with mean age of 56.8 years (21-78 years) enrolled. The program was completed by 29/30 patients (97%), 1 did not complete the program and 11 remain in the program. All patients completed the outcome measures. Of the patients who completed the program 14/29 (48.3%) met their goal (9 joint replacements, 5 optimized LBP). Average patient satisfaction was reported as “excellent”. Conclusions: An intensive lifestyle medicine program is feasible for patients seeking improved management of their musculoskeletal disorder while also improving overall health. This care model needs further investigation and physiatrists are well suited to serve as coordinators for these programs.
