Objective: To test the hypothesis that functional outcomes following discharge from acute inpatient rehabilitation (IRF) are superior in patients with refractory epilepsy receiving laser ablation compared to surgical resection

Design: Retrospective non-consecutive case series Setting : Midwestern large academic acute inpatient rehabilitation facility Participants : 8 patients in IRF status post ablation (n=3) or resection (n=5)

Interventions: IRF following ablation or resection

Main Outcome Measures: Functional Independence Measure (FIM) total change, FIM efficiency, FIM change subset measures of memory and problem solving, seizure incidence

Results: Comparing patients admitted to IRF who have received ablation with patients who received resection, there was was no statistically significant change in total FIM change (mean 43.7 vs 36), FIM efficiency (mean 5.3 vs 2.2), or FIM change subset measures of memory (mean 0.5 vs 0.25) or problem solving (mean 0 vs 0.8). There was no statistical significance between groups in having a seizure (n=0 vs n=2) Conclusions: No statistically significant difference in functional outcomes following discharge from IRF in patients with refractory epilepsy who received ablation as compared to resection were discovered in this case series. However, a larger sample size is necessary before generalizations may be made.
