Objective: Assess the indications and efficacy of ultrasound-guided erector spinae block (ESP) for pain treatment in a recently-structured interventional pain clinic.
Design: Retrospective case series
Setting: Interventional Pain Clinic at a private insurance responsible for over 400,000 lives
Participants: Individuals that underwent ESP from October/2018 to February/2019 were identified: eight lumbar and two thoracic injections. Thoracic pain was secondary to post-herpetic neuralgia. Lumbar pain was secondary to radicular pain due to central/foraminal stenosis (four cases), chronic myofascial pain with central sensitization (two), failed back surgery syndrome (one).
Interventions: Ultrasound-guided ESP with 10 mL of 0.5% ropivacaine and dexamethasone 4 mg in each side.
Main Outcome Measures: Visual analog scale (VAS) for current pain, Oswetry Disability Index (ODI) for low back pain.
Results: Ten individuals were identified: three males and seven females, with age 68.2 (8.82). ODI at baseline was 25 (10.6). VAS decreased from 8.0 (2.1) to 3.5 (3.1) after the procedure, with median (range) VAS decreasing from 8.0 (5.0-10.0) to 2.5 (0.0-8.0). Six individuals presented >50% pain relief. No adverse events were observed
Conclusions: ESP provided immediate pain for thoracic and lumbar pain without adverse events. Duration of the effect is still being assessed by our group.
