Objective: To understand the effects of commonly used local anesthetics on the cellular composition of platelet-rich plasma injections.
Design: Leukocyte rich PRP (LR-PRP) was prepared using a commercial product (Harvest TerumoBCT) from 30 mL whole blood and spun down in a double spin protocol to yield a 4 mL LR-PRP product. The LR-PRP was validated using platelet counts determined by scatter cytogram and platelet integrated analysis (ADVIA 120 hematology analyzer). 475 microliters of LR-PRP was combined with 25 microliters of each anesthetic or control: lidocaine, bupivacaine, normal saline, or dextrose to make four separate solutions. Various cell line counts were then quantified and compared. Red and white blood cell analysis was done by volume histogram and integrated analysis (ADVIA 120 hematology analyzer).

Participants: N/A
Interventions: The anesthetics or controls of lidocaine, bupivacaine, normal saline, or dextrose were added to separate solutions containing LR-PRP.
Main Outcome Measures: Platelet count, WBC profile, and the RBC count were measured before and after the addition of the solutions.
Results: The results did not show a significant change in the platelet count, WBC profile, RBC count, or large unstained cell count with the addition of lidocaine, bupivacaine, normal saline, or dextrose. Notably, the cell counts showed no significant change even between the varying concentrations of lidocaine and bupivacaine. Due to a small sample size, further studies may be required to adequately and reliably demonstrate more representative data.
Conclusions: As past studies have demonstrated a mitigative effect of local anesthetics on the efficacy of PRP, it is interesting to find that such anesthetics may not significantly alter the cellular composition of PRP in vitro. Further study may be required to better understand the effects of anesthetics on PRP efficacy if they do not alter its cellular profile.
